Our managers help define what well-being means to you personally and then work to identify and coordinate the right resources to help you get there. The goal is to help you learn the key steps to managing your own health care.

THE TEAMS WILL:
Assist with making and keeping appointments

Help you communicate with your care providers

Help you access information about your health care needs

Help in obtaining benefits

Answer questions about your medical and/or behavioral health

Link you to housing, transportation, and food resources

Connect you with other community resources
